ELDA Forms
Every two years, ELDA holds a general conference of its members with member churches taking turns hosting the conference at their locations.  During the conferences the ELDA Board honors individuals and churches or ministries that have demonstrated excellence in service in line with the mission and goals of ELDA. 

These awards are the "Shining Light Award" given to individuals and the "City on a Hill Award" which is given to member churches or ministries.  You can fill out the nomination forms online, or you can print the PDF versions of the award nomination forms to give to members who do not have access to computers but would like to nominate a person or a church/ministry.
